• All primary care office visits including routine care, urgent care, and an annual physical exam, all during our regular office hours
• Same-day appointments with minimal wait times
• All in-office diagnostic tests and procedures
• Routine injectables and annual flu vaccine
• Email & phone consultations
• Coordination of Specialty Care

Any necessary lab or radiology studies that are ordered will be outsourced to service providers who will bill your health insurance accordingly.

Our Gold Plan is ideal for our patients who understand the ‘consumer direction’ that healthcare is taking. The Gold Plan includes:

• All primary care office visits including routine care, urgent care, and an annual physical exam, all during our regular office hours
• Same-day appointments with minimal wait times
• All in-office diagnostic tests and procedures
• Routine injectables and annual flu vaccine
• Email & phone consultations
• Coordination of Specialty Care
• $450 laboratory allowance
• $300 radiology allowance

The Platinum Plan is our all-inclusive plan, extending our highest levels of service and access, and is designed for our patients who insist on a superior level of personalized care. With a further focus on prevention and wellness, this plan includes an annual executive physical exam in addition to comprehensive primary medical care. The Platinum Plan includes:

• All primary care office visits including routine care, urgent care, and an annual physical exam
• Same-day appointments with minimal wait times
• All in-office diagnostic tests and procedures
• Routine injectables and annual flu vaccine
• Email & phone consultations
• Coordination of Specialty Care
• $600 laboratory allowance
• $400 radiology allowance
• USB medical records keychain
• 24/7 access via physician's cell phone
• House/Office calls when appropriate
